Terms of Participation

1. Players in the Humber Valley Old Timers, must behave in a sportsmanlike manner, with due respect for other players, officials and league executives and abide by the CODE OF CONDUCT rules of the HVOT.

2. ALL PARTICIPANTS MUST COMPLETE THE ONLINE 'CODE OF CONDUCT' FORM PRIOR TO THE START OF THE SEASON.

3. PLAYERS MUST ATTEND 60% (12 games) OF THE REGULAR SEASON GAMES TO MAINTAIN PARTICIPATION IN THE FOLLOWING SEASON AND WILL BE PLACED ON THE SUBSTITUTE PLAYER LIST! (Injuries excluded).

4. No body contact, no slapshots, and standard old timer rules apply.

5. The player releases from liability and agrees that he shall not hold responsible the Humber Valley Old Timers, any HVOT Executive or City of Toronto or any individual acting through them and also acknowledges that they do not assume any responsibility for any injury, damage, or loss resulting from an incident or accident connected with participation in any HVOT activity or from any known or unknown condition, handicap, infection, virus or from any cause whatsoever.

6. Players must monitor their own physical condition and health to ensure playing safety & capability.

7. Full protective equipment must be worn at all games including CSA approved helmets. Use of full face protection is strongly recommended: half visor is the minimum accepted requirement.

8. Submitting this player renewal application does not allow you to participate in the HVOT league.
