Liability and Photo Release:
As additional consideration given for my son, daughter or ward ("camper") to participate in a Bethel Tate High School program Camp ("Camp"), I, the undersigned, (a) verify that my Camper is physically fit to participate in the strenuous athletic activity at the Camp; (b) release the Bethel-Tate High School soccer program, Bethel Tate Schools, individual sports camp and all operators, members, coaches, trainers, contractors, employees, volunteers and sponsors (collectively, the "Camp Operations") from any and all damage or loss to my Camper related directly or indirectly to the camp. I also authorize any representative of the Camp to act for me according to their best judgment in an emergency requiring medical attention for my Camper.
Bethel-Tate Local Schools has my permission to use my or my child's photograph publicly to promote Bethel-Tate Local Schools/Bethel-Tate Youth Soccer Camp. I understand that the images may be used in print publications, online publications, presentations, websites and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
