Hamilton Northern Burlington Babe Ruth League
Parent and Player Agreement

I hereby state that my child resides/attends school within the Hamilton Northern Burlington Babe Ruth territory and want to participate in the Hamilton Northern Burlington Babe Ruth League Baseball Program. I agree to abide by the rules and regulations of the league and will play with the team to which I may be assigned. I further agree to protect all League property and to return same when required. I understand that the Sponsors, Managers, Coaches and League Officials shall not be responsible for any injury I may sustain while I am participating in the activities of the league.

I agree to the fees and my intended commitment to my child's participation in the league.

Submission of the registration form either online, by mail or to league official acts as the signature of Parent/Guardian.

IMPORTANT NOTE: New players must provide player agent with a copy of child's birth certificate and proof of residency either by mail or in person prior to try outs.
