By registering, players and parents acknowledge and agree to the following:

- Participation on this team requires commitment, attendance, communication, teamwork, and sportsmanship.
- Players are expected to make a good-faith effort to attend practices, games, and team activities.
- Families must communicate absences, conflicts, or schedule changes as early as possible.
- Parents/Guardians agree to disclose participation in other teams, leagues, sports, or recurring activities that may conflict with team commitments.
- Once game availability is confirmed, families are expected to honor that commitment except in cases of illness, injury, family emergency, school obligations, or other unavoidable circumstances.
- Repeated unexcused absences, disruptive behavior, poor sportsmanship, or failure to communicate may impact playing time, roster status, or continued participation on the team.

WAIVER INFORMATION

In consideration of my child participating in Parkchester Little League related events and activities, the undersigned acknowledges that the risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury will always exist.

Realizing that risks cannot be eliminated, I agree to the following:

I, FOR MYSELF, SPOUSE, AND CHILD, AND TO THE GREATEST EXTENT ALLOWED BY LAW, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS OF INJURY TO MY CHILD, both known and unknown and, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (as defined below) or others (including fellow players), and assume full responsibility for my child's participation.

I willingly agree to comply with the program's stated and customary terms and conditions for participation.

I HEREBY RELEASE AND HOLD HARMLESS Parkchester Little League, its directors, officers, officials, agents, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the Program (referred to in this Agreement as ?Releasees?), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property arising out of or related to my child's involvement or participation in these Programs.

CODE OF CONDUCT ? Zero Tolerance Policy

Players, parents / guardians, and spectators are expected to demonstrate respectful behavior, positive sportsmanship, and appropriate conduct toward coaches, teammates, opponents, officials, and other families at all times, including online and at team events. Any parent / guardian, player, or spectator found to be unruly, violent, threatening, abusive, or instigating violence or fighting ? verbally or physically ? may be asked to leave the premises immediately and may be expelled from participating in Parkchester Little League for the remainder of the season. No refunds will be provided.
Al registrarse, los jugadores y padres/madres o tutores reconocen y aceptan lo siguiente:

* La participaci?n en este equipo requiere compromiso, asistencia, comunicaci?n, trabajo en equipo y buen esp?ritu deportivo.
* Se espera que los jugadores hagan un esfuerzo de buena fe para asistir a pr?cticas, juegos y actividades del equipo.
* Las familias deben comunicar ausencias, conflictos o cambios de horario lo antes posible.
* Los padres/madres o tutores aceptan informar sobre la participaci?n del jugador en otros equipos, ligas, deportes o actividades recurrentes que puedan entrar en conflicto con los compromisos del equipo.
* Una vez confirmada la disponibilidad para los juegos, se espera que las familias cumplan con ese compromiso, excepto en casos de enfermedad, lesi?n, emergencia familiar, obligaciones escolares u otras circunstancias inevitables.
* Las ausencias repetidas sin justificaci?n, el comportamiento disruptivo, la mala conducta deportiva o la falta de comunicaci?n pueden afectar el tiempo de juego, el estado en el roster/equipo o la participaci?n continua en el equipo.

# INFORMACI?N DE EXENCI?N DE RESPONSABILIDAD

En consideraci?n de la participaci?n de mi hijo/a en eventos y actividades relacionadas con Parkchester Little League, el abajo firmante reconoce que el riesgo de lesi?n para mi hijo/a derivado de las actividades involucradas en estos programas es significativo, incluyendo la posibilidad de discapacidad permanente y muerte, y que aunque ciertas reglas, equipos y disciplina personal pueden reducir este riesgo, el riesgo de lesiones graves siempre existir?.

Reconociendo que los riesgos no pueden eliminarse, acepto lo siguiente:

YO, POR M? MISMO/A, MI C?NYUGE Y MI HIJO/A, Y HASTA EL M?XIMO PERMITIDO POR LA LEY, ASUMO CONSCIENTE Y VOLUNTARIAMENTE TODOS LOS RIESGOS DE LESI?N PARA MI HIJO/A, tanto conocidos como desconocidos, INCLUSO SI SURGEN DE LA NEGLIGENCIA DE LAS PARTES EXONERADAS (seg?n se define m?s adelante) u otras personas (incluyendo otros jugadores), y asumo plena responsabilidad por la participaci?n de mi hijo/a.

Acepto voluntariamente cumplir con los t?rminos y condiciones establecidos y habituales del programa para la participaci?n.

POR LA PRESENTE, EXIMO Y LIBERO DE RESPONSABILIDAD a Parkchester Little League, sus directores, oficiales, funcionarios, agentes, voluntarios, otros participantes, agencias patrocinadoras, patrocinadores, anunciantes y, si corresponde, a los propietarios y arrendadores de las instalaciones utilizadas para llevar a cabo el Programa (referidos en este Acuerdo como las ?Partes Exoneradas?), CON RESPECTO A CUALQUIER LESI?N, DISCAPACIDAD, MUERTE, p?rdida o da?o a personas o propiedad que surja de o est? relacionada con la participaci?n o involucramiento de mi hijo/a en estos Programas.

# C?DIGO DE CONDUCTA ? Pol?tica de Cero Tolerancia

Se espera que los jugadores, padres/madres o tutores y espectadores demuestren comportamiento respetuoso, buen esp?ritu deportivo y conducta apropiada hacia entrenadores, compa?eros de equipo, oponentes, ?rbitros y otras familias en todo momento, incluyendo en l?nea y durante eventos del equipo.

Cualquier padre/madre o tutor, jugador o espectador que sea considerado desordenado, violento, amenazante, abusivo o que incite violencia o peleas ? verbal o f?sicamente ? podr? ser retirado inmediatamente de las instalaciones y podr? ser expulsado de participar en Parkchester Little League por el resto de la temporada. No se otorgar?n reembolsos.
