2022 PLAYER REGISTRATION FORM



CONSENT AGREEMENT

In consideration of acceptance into the Granite State Baseball League I agree to conduct myself in a manner which will reflect favorably upon my teammates, fellow competitors, umpires, league officials and spectators. I understand that
failure to conduct myself in accordance to proper sportsmanship, the league rules and the league conduct code may result in my suspension without reimbursement of any fees I may have paid. I have received and read the by-laws, rules and regulations of the Granite State Baseball League and agree to abide by them at all times.

I understand that certain risks are inherent in my participation in the game of baseball and I assume those risks of my own accord and will not hold the Granite State Baseball League and its officials, umpires, sponsors and field owners responsible of any injury I may sustain in the course of traveling to and from, or while participating in any league activity.

In signing this form I agree that I shall complete the season on the team for which I originally registered unless, after completing and submitting to the President the GSBL ?Player Changing Team Form? attesting to the mutual consent
among himself, his current team manager and the receiving team manager, I am moved or traded to another team.

I have read, understand and accept all of the above and certify under penalty of long term suspension that my birthdate and age are correct as listed above.

NOTE: A copy of your driver's license is required for all 1st year players or returning players who do not have a copy on file with league.

For 2022, due to the merger, we are requesting all players to resubmit a picture ID to the league.
