By hitting the SUBMIT FORM button you are starting an Appeal of your suspension, and agreeing to the guidelines set forth on the previous page (*Forms Tab*Suspension Appeal Process), including your team's owner is responsible for $100 to the PVTL. If the suspension is overturned the $100 will be returned to your team's owner. Your team's owner will be contacted immediately upon receipt of this email to the PVTL.
