MEDICAL HISTORY/CONDITIONS:

IMPORTANT WCFL CHEERLEADING WAIVER
I am the natural parent (or legal guardian) of the aforementioned child, who is in my custody under the age of 18 years old. The information on this form, the birth certificate are accurate.

My child will abide by the rules of the Western Communities Football League, Inc. (WCFL). Said child, with my consent, is participating as a football player or a cheerleader in the WCFL, Football/Cheerleading program, sponsored by the WCFL.

I hereby authorize the WCFL to provide transportation, if applicable for special events, for said child and waive any claims that my child and/or I might have on behalf of my child for injuries received while being transported to and from said “away” football games or special events.

It has now been explained to me and I do understand that Football/Cheerleading is a violent contact sport.

I understand that my child may be physically injured, paralyzed, or even death may occur.

I consent to my child’s participation in the WCFL program sponsored by the named organization. In the event that my child is so injured, I hereby request that said child receive emergency medical treatment and therefore authorize the WCFL to obtain a physician licensed by the State of Florida and/or member of a county Emergency Medical Service Unit, as well as, any hospital in the State of Florida, to treat my child for injuries sustained while playing football or cheerleading.

I agree that no municipality, employee, member, sponsor, coach, or participant of the WCFL will be held responsible for any injury to my child while participating in any activity of the WCFL Program.

I understand that the insurance policy which is provided by the WCFL is excess coverage insurance and the insurance pays only those hospital and doctor expenses which are not covered by any health or hospital insurance for the above named, in effect at the time of the injury.

I agree that all such claims for the above child will be submitted to the primary health insurance carrier by which the said child is covered, and I agree that only claims for expenses not covered by such health or hospital insurance will be submitted to the WCFL. Anyone participating in the program is covered by a $250.00 deductible excess coverage policy. The first $250.00 will be paid by the parent or legal guardian.

I agree upon request of the WCFL that the uniform and other equipment issued to the above named child will be returned in as excellent/good condition as received, except for normal wear and tear. In the event that any of the above named equipment is not returned upon request, I agree to accept financial responsibility and liability for all unreturned equipment up to a maximum of $350.00 for football players and $100.00 for cheerleaders.

If legal expenses are incurred by the WCFL in order to recover any equipment of the above said child, I agree to reimburse the WCFL for such costs. There will be NO REFUNDS AFTER May 1st (no exceptions).
WCFL PARENT/COACH CODE OF CONDUCT
I hereby pledge to provide positive support, caring and encouragement to the WCFL, my child, other players and cheerleaders, the WCFL Board of Directors, the coaches and fellow parents of this League by following this Parents Code of Conduct for the season.

*I will encourage good sportsmanship by demonstrating positive support for all participants, Coaches, Officials, Board Members and Staff in all of the WCFL’s sponsored events.

*I will place the emotional, psychological and physical well being of my child and any other child in the WCFL ahead of any personal desires.

*I will treat each participant as an individual, remembering the large range of emotional and physical development for the same age group.

*I will promise to review and practice the basic first aid principles needed to treat the injuries of the participants.

*I will do my best to organize practices that are fun and challenging for all of the participants.

*I will lead by example in demonstrating fair play and sportsmanship for all of the participants.

*I will use coaching techniques that are appropriate for all of the skills that I teach.

*I will demand that my child participates in a safe and healthy environment, which is a drug and alcohol free at all League sponsored events.

*I will support all of the Coaches, Board Members and other volunteers working with my child in order to encourage a positive and enjoyable experience for all.

*I will do my best to make the WCFL fun for my child and will remember that the League is for the children and not the adults.

*I will ask my child to treat other Participants, Coaches, Board Members, Parents and Spectators with respect regardless of race, sex or ability.

*I will refrain from vulgar, lewd or obscene language or gestures while participating in or watching any League sponsored event.

*I will direct all of my comments and criticisms in a calm and civilized manner to the Board Members, Coaches, OR Game Officials away from the children participating in the League.

*I will remember that I am a youth sports coach and/or parent, and that the game is for the children and not for the adults.

I have read and agree to abide by the Parents Rights and Responsibilities, better known as the Code of Conduct and will agree by clicking on "I agree", which acknowledges these guidelines.

PLEASE NOTE: In order to participate in the WCFL, all parents must agree and the date this registration is submitted acknowledges that these Codes of Conduct have been read and will be followed. Failure of the parent to read and agree to the Parents Code of Conduct will automatically render that parent or coach ineligible to participate in any WCFL league activity (the child may still participate).

In addition, by agreeing to this Code of Conduct, parents pledge to abide by the guidelines set forth above and to conduct themselves in an appropriate manner. Failure to abide by this Code of Conduct, or violation of any of the guidelines set forth above, will automatically render the parent or coach unable to participate or attend any WCFL league sponsored event.

The Western Communities Football League, Inc. Board of Directors has the sole right to enforce violations; however, the parent may appeal through the Disciplinary Appeal Process.

I understand any violation of the Parent Code of Conduct can result in my removal from the WCFL Football Complex, Parking Lot and program.
WCFL TERMS, POLICIES AND CONDITIONS GUIDELINES
1. It is understood that no refunds will be issued after May 1st for cheerleading.
2. All registrants after the close of final advertised registration day shall be subject to a $35.00 late fee, payable in CASH ONLY with said registration fees.
3. Games are tentatively scheduled for SATURDAYS ONLY, but due to weather may be changed to weeknights, if applicable.
4. Teams are subject to two nights of practices a week plus any additional practices for special events.
5. The WCFL operates its Cheerleading season from late May until the first part of December. The Village of Wellington shall govern all field schedules and closures for Holidays, weather, or acts of God.
6. It is understood that the WCFL is a youth football and cheerleading program and as such operates with and by the efforts of Community volunteers. These volunteers are here for the children and are not subject to ridicule, or abusive language or behavior by anyone. Should you engage the volunteers in this manner you or your family will be asked to leave the WCFL Football Complex and/or the WCFL program.

Parents will be responsible for all cheerleading uniform expenses. Registration does NOT cover the cost of uniforms or accessory items for the season.

Please note that during the first week of practice, the last Tuesday in July, your cheerleader will go home with paperwork that will be due back no later than the third week of practice. You will be required to also turn in a wallet-size photo of your cheerleader and a copy of your child's birth certificate. Failure to turn in these items by the third week of practice renders your child as ineligible to participate.

Payment is required in FULL at time this registration is submitted. If you submit the form without payment, you will be emailed and asked to show to one of our "LIVE" registrations to pay in CASH in FULL.
