WIAA CONCUSSION AGREEMENT FOR PARENTS AND PLAYERS

Parents and Players: Please review the WIAA handouts provided on this website prior to registration.

I, a parent/guardian of the above student athlete participant in the Barron Youth Wrestling Association, hereby acknowledge having received education about the signs, symptoms, and risks of sport related concussion. I also acknowledge my responsibility to report to my coaches, parents(s)/guardians(s) any signs or symptoms of a concussion. I certify that I have read, understand, and agree to abide by all of the information contained in this document, I have sought and received an explanation of the information prior to agreement to this statement.

I, as parent/guardian of above player, agree with the above WIAA Concussion statement and I am in agreement. *

I understand as a parent/guardian that there are more cost involved that I will be
responsible for: (Weekend tournament cost, travel and other wrestling apparel.)

I, as a parent/guardian of above participant, also understand that during the season there will be fundraising events and events which I will be asked to work or donate items.

I, as a parent/guardian of above participant, also will give general support of the Wrestling Booster Club.

I, as a parent/guardian of the above participant acknowledge and accept that neither the School District of Barron Area Schools, Barron Area Booster Club nor any coach or persons associated with the Barron Area Booster Club are or is responsible or liable in any way of injury or accidents experienced by my child. I knowingly and voluntarily assume all risks of loss and all legal and financial responsibility therefore.

(Register first child at $30, second child at $20, and 3+ registration is $0. Then submit the registration form. Then you can select Add Registration if appropriate.
