I consent to my child(ren)'s participation in the CTK HEC Athletics Program. I understand that no athletic program, including this one, is without risk. In particular, I understand that playing sports can result in injury. Nevertheless, by registering and participating in the CTK HEC Athletics Program, I agree to assume all liability for the risk of injury, illness or death on account of myself and my dependent's involvement in the CTK HEC Athletics Program whether caused by negligence of the organizers or another person on the premises or at the sponsored activity.
