Volunteer/Coaching Form

USA Softball requires an Annual Background Check Certification in addition to Safe Sport training and certification to be completed by all volunteers participating in any local softball organization. In order to remain compliant, please complete the below form to initiate the enrollment process for the current season. New members will be registered by the league and you will receive your online credentials. Existing members should log in to their individual USA Softball Account at registerusasoftball.com and renew their annual background check.

If you are an existing USA softball member, please include your member number in the below form if you have it. If you are a returning volunteer, and have an existing USA softball profile, please upload any state required clearance forms to your profile with USA Softball or you can also email them to dgsaleague@gmail.com.

These include: [1] PA State Police Criminal History Check, [2] PA Department of Human Services Child Abuse Check, and [3] FBI Clearance (only for those who have not lived in the State of PA for the past 10 years)
