COVID-19 has been declared a worldwide pandemic by the World Health Organization. PARKCHESTER LITTLE LEAGUE has established preventative measures and policies to reduce the risk of spread of COVID-19. However, cannot, guarantee that you, your child(ren), will not become exposed to and infected with COVID-19.
This Waiver and Release of Liability Relating to Coronavirus/COVID-19 (Waiver and Release) is effective for the entire 2024 baseball season and encompasses any and all Parkchester LL events and activities I acknowledge and understand the contagious nature of COVID-19, and I voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by participating in Parkchester LL activities
In consideration of my child participating in PARKCHESTER LITTLE LEAGUE related events and activities, the undersigned acknowledges that the risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury will always exist. Realizing that risks cannot be eliminated, I agree to the following: I, FOR MYSELF, SPOUSE, AND CHILD, AND TO THE GREATEST EXTENT ALLOWED BY LAW, KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS OF INJURY TO MY CHILD, both known and unknown and, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (as defined below) or others (including fellow players), and assume full responsibility for my child's participation; and, I willingly agree to comply with the program's stated and customary terms and conditions for participation. HEREBY RELEASE AND HOLD HARMLESS PARKCHESTER LITTLE LEAGUE its directors, officers, officials, agents, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the Program (referred to in this Agreement as "Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property arising out of or related to my child's involvement or participation in these Programs, Code of conduct Zero tolerance policy Any parent or ball player found to be unruly, violent or instigating violence/ Fighting Verbally or physically Will be asked to leave the premises and Will be expelled from participating in Parkchester Little League for said season No refunds will be given. I HAVE READ THI S RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
