2019-20 Rec Basketball Registration Form


The township mandates all coaches have the following to participate in the program:

1- Rutgers certification
2- Fingerprinted and have passed a background check
3- Complete concussion certification

Participation in the program is limited to those children interested in playing basketball. Involvement in any other sport that may hinder participation in this program, to the detriment of the remaining players on the team may result in the expulsion by vote of the Board of Directors. If a parent is aware of a possible conflict, he or she should notify the Board of Directors at the time of registration in order to resolve such a potential conflict.

Parental Consent:
I, the parent and/or legal guardian of the above named registrant, a minor, do hereby give my permission for said registrant to participate in any and all basketball related activities and both I, on my behalf and on behalf of the registrant, agree to abide by the rules of River Vale Basketball Association. I recognize that basketball is a physically demanding sport and I certify that the registrant is in good, sound physical condition, free from illness, injury, or disability, which would endanger their health or render it difficult or impractical for such child to participate in such an activity. I recognize that basketball is a contact sport and that there is a possibility of physical injury associated with basketball and in consideration for the aforementioned Association accepting the registrant in its program, I do hereby release, discharge, absolve, indemnify and agree to hold harmless River Vale All-Star Basketball Association, its organizers, sponsors, supervisors, officers, coaches, coaching staff, and employees, associated personnel, including without limitation the owners of courts and/or facilities utilized for the program, participants and persons transporting such registrant to, from or during such activities from any and all manner of claim arising out of injuries or otherwise to registrant whether as a result of negligence or for any other cause.

Medical Care:
As parent or legal guardian does hereby give his/her consent for emergency medical care prescribed by a duly licensed physician or dentist in the event such emergency medical care is required for the health, safety or welfare of the registrant due to illness or injury at a time the parent or legal guardian in not available.


This Code of Conduct applies to all parents/guardians/guests of athletes involved in River Vale Basketball youth sports activities


1. TRUSTWORTHINESS – Be worthy of trust in all you do. Be loyal to your child’s team and to the whole youth sports organization. Live up to high ideals of ethics and sportsmanship. Teach your children the importance of integrity, honesty, reliability and loyalty.

2. RESPECT – Treat all people with respect at all times and require the same of your children. Treat game officials with respect. Don’t complain or argue about calls or decisions during or after an athletic event. Teach your children to treat other players, coaches, officials and spectators with respect regardless of race, creed, color, gender or ability.

3. RESPONSIBILITY – Consistently exhibit good character and conduct yourself as a positive role model. Exercise self-control; avoid displays of anger and frustration. Don’t retaliate.

4. FAIRNESS – Live up to high standards of fair play. Treat all competitors fairly. Be open-minded.

5. CARING – Offer positive reinforcement. Consistently demonstrate concern for athletes as individuals and encourage them to look out for one another and think and act as a team.

6. CITIZENSHIP – Honor the spirit of the rules. Teach your children to avoid temptations to gain competitive advantage through improper techniques that violate the highest traditions of sportsmanship.


1. I will respect the rules of the game and the policies of RVBA/RVAB and will abide by the core values outlined above.

2. I will respect the coaches/officials and their authority before, during and after games and will never confront officials and/or coaches before, during or after the game. If I have a concern, I will take time to speak with my child’s coach at an agreed upon time and place. If I have further concerns beyond my conversations with the coach, I will contact a League Director.

3. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.

4. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player or parent such as booing and taunting; or using profane language or gestures.

5. I will not encourage any behaviors or practices that would endanger the health and well-being of the athletes. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence.

6. I will never ridicule or yell at my child or other participants for making a mistake or losing a competition.

7. I understand RVBA/RVAB events are free from drugs and alcohol and I will refrain from their use at all sports events.

8. I will refrain from coaching my child or other players during games.

III. Penalty


That if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary action that could include, but not be limited to, the following:

a. Verbal warning by Official, Head Coach, League Director or other Officer of RVBA/RVAB.
b. Multi game suspension from attending RVBA & RVAB sports
c. Outright ban from attending RVBA /RVAB sports activities.

1. Any spectator who receives one (1) warning (at any time during the season) that his/her behavior violates the spirit of this code during any RVBA/RVAB contest will be asked to exit the venue immediately upon the occurrence of a second misconduct violation. Any violation of the rule will result in an immediate removal of the spectator’s child from the contest and/or forfeiture of the game. THIS MEANS YOU ONLY GET ONE WARNING PRIOR TO BEING ASKED TO LEAVE THE EVENT.

2. Any spectator who is asked to exit the venue agrees to an immediate two game suspension. This means they may not attend any RVBA sporting events until such suspension is completed. Any violation of this rule will result in an immediate removal of the spectator’s child from the contest and/or forfeiture of the game.

3. I agree if I am asked to leave any sporting event for violation of the aforementioned parental code of conduct that I may be subject to a year- long suspension from attendance (including coaching) at any RVBA/RVAB sponsored activity. I agree that such determination rests solely with the RVBA and RVAB Board and its officers.

4. I understand there is no appeal.
