ATTENTION:

Only families that have been invited to join Urbana Rippers Travel Softball Teams are required to register via this form. As part of accepting your invite to play on the team we will require a non-refundable down payment of $500 which is the first installment of payments for this team.


Registration Dates: You must accept your invitation within 72 hours of being notified of your daughter making the team.

Refund Policy
I understand that NO refunds under ANY circumstances will be given for Invitation Acceptance registration. I understand that the acceptance of this invitation is a commitment to play and stay on the team for the complete season.

Acceptance of Team / Player Likeness and Name Sharing
Travel team accomplishments, team photos, player's names, and individual player related photos MAY be used by the league for the promotion of the team and the league in general within all league related marketing, web, and public relations materials. I acknowledge, as the legal guardian and parent of a travel player that I understand and accept this possibility and I am giving permission to use my players information for this purpose.

Waiver & Consent to Treat
Parents will be attempted to be notified in the case of a serious injury or illness. This form makes immediate treatment possible for your player and is required for your player to participate in the Urbana Baseball & Softball league.

As parent/guardian of (the person being registered for this sport), I electronically authorize participation in all activities of the above program. I assume all risks and hazards of such participation both during an activity and en route, and do hereby release and waive all claims against Urbana Baseball & Softball, its board, staff, volunteers, participants, Frederick County Division of Parks and Recreations, Frederick County, its officers, agents, associations, employees, or any person or entity connected with the team, organization, or facility. I also grant permission for emergency first aid to be given to my child in case of an injury or emergency medical situation.


Alcoholic Beverage Policy for Urbana Rippers Travel Softball
Urbana Softball has instituted a new alcohol policy for coaches and parents of our program according to the following guidelines:

- No alcoholic beverages are allowed at ANY games regardless of location.

- Parents (or coaches) who are found to be in possession of alcohol and deemed to be disruptive to either the game, coaching, players, or opposing team will be subject to disciplinary action which includes a 3 game suspension for them and their player for the 1st offense and a removal from the team for a 2nd offense.

- Post game activities and the consumption of alcohol by parents and coaches should be done in moderation. The league has complete disciplinary discretion to take whatever actions they feel are prudent for issues that arise outside games which are influenced by alcohol consumption.

Urbana Baseball & Softball Return to Play Waiver

As parent/guardian of the person participating in the Urbana Baseball & Softball program, related events and activities, I acknowledge and agree that:

1) The risk to have contact with individuals, who have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to COVID-19 does exist, and it is impossible to eliminate the risk that the person participating could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease.
2) I, on behalf of the person participating knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of anyone involved with Urbana Baseball & Softball, and voluntarily elect to participate in all related baseball and softball activities.
3) I hereby release and hold harmless the Urbana Baseball & Softball program, their officers, officials, volunteers, agents and/or employees, and other participants with respect to any and all injury, illness, disability, death, loss or damage to person or property, whether arising from the negligence or otherwise.
