ball hit to right field, runner from first is played on at second, ball is overthrown into dead ball area. Runner from first going to second misses the bag and continues to third. What is the responsibility of the umpire, the runner, the defense?

Alex umpire - know exactly where runner was when ball was thrown/released. Did runner miss base.Call dead ball when out of play. Rule if defense appeal of runner missing second base. Runner cannot go back and touch missed base after dead ball declared.ASA rule 7-I-3. Defense - appeal before next pitch.
