With a runner at 3rd base, a pitched ball is missed by the catcher and hits the umpire's ball bag knocking out a spare ball behind the catcher and umpire. Now there is the game ball and a spare ball behind home plate and the catcher is confused as to which ball is live but picks up the ball closest to her to attempt to make a play.
Runner breaks for home on the passed ball and scores.

Ball or strike called, then call dead ball, and return the runner.
