Tournament Roster Form
Coaches must submit this roster form in order to participate in the tournament. They must be sent to Alberto Ariza via email: albertoariza2@gmail.com by March 15th, 2021.

No player signature is required as long. Your roster should have the player's name, date of birth, and jersey number.

Coaches MUST bring physical copies of birth certificates on the day before the team's first game and present them to a tournament director. Failure to comply to this rule will result in a forfeit for the team's first game.
