Player/Parent Commitment Agreement
All Players must sign the Agreement before being accepted as a new or continuing member of the Chippewa Valley Raptors. Please print the form, sign it, and return it to your coach or email to info@chippewavalleyraptors.com.

Consent for Treatment and Release of Liability
All athletes playing for the Chippewa Valley Raptors must have a Consent for Treatment of Minors and Release of Liability form on file prior to participating in any team practice, game, tournament, or team building activity.

Parent/Guardian and Player Concussion Agreement
All athletes playing for the Chippewa Valley Raptors must read the Player Concussion Fact Sheet along with the Facts about Concussion and Brain Injury Sheet, and have a Parent/Guardian and Player Concussion Agreement Form on file prior to participating in any team practice, game, tournament, or team building activity.

These Sponsorships help cover registration, tournament fees, uniforms, equipment, training facilities, team building activities and travel costs that allow young ladies around the Chippewa Valley to compete at the highest level of fastpitch softball.
