17 Physical Form
All football players must have a current physical that does not expire during the season to participate. Registration packet includes: Player Registration, Player Code of Conduct, Parent Code of Conduct & Player Physical Form.

Craven Jaguar Volunteer Form
Please fill out completely. Any additional pertinent information may be attached. All volunteers are subject to a National Background check. Each year the league provides staff, coaches and volunteers with a link to obtain a background check at a much more reasonable fee. See East Carolina Youth Football League website for update or Ask staff for assist to obtain.

17 Coach Application
Coach Application must accompany Coach Code of Conduct form. A National background check is required for all staff members. In addition, certifications must be completed by deadline. See Coach need article for details.
