Referee Misconduct Report must be mailed to Mesquite Soccer Association, 309 N. Galloway Ave., Suite #104, Mesquite TX 75149 Attn: SGT at ARMS - and mailed within 48 hours of the game.

International Clearance Process - Form PRIOR TO 10 (P10 5-11)
Players with foreign birth certificates (U4 and above) must have international clearance. This is the 1st of 3 forms required to be completed and sent to the Registrar. No player can register and/or play without clearance first from the US Soccer Federation. Therefore, you MUST have the parent complete this form ASAP to avoid any delay or risk or not being eligible to play in this season. Fall registration -
A copy of the players birth certificate and US Shot Records are required for verification..

International Clearance Process - Form FIRST REGISTRATION (FR-11)
Players with foreign birth certificates (U4 and above) must have international clearance. This is the 2nd of 3 forms required to be completed and sent to the Registrar. No player can register and/or play without clearance first from the US Soccer Federation. Therefore, you MUST have the parent complete this form ASAP to avoid any delay or risk or not being eligible to play in this season. Fall registration -
