Please fill out pages 1-6. Page 6 includes a Physical Exam evaluation that must be completed by your Health Care Provider after June 1, 2015 - All prospective players must fill out.

For all athletes that intent to play a sport during the school year need to fill out and declare - All prospective athletes need to complete.

SAHS ATHLETIC FEE Form (Rostered Players only)
To be completed by rostered players following tryouts and covers all Sports played during the school year.

Team and Individual Picture Order Form
Picture Day - SEPTEMBER 19 starting at approx. 8:30 AM (pictures to be taken before the home game that day)
