Lists all the players on the team's roster. Should be submitted with the player contract when the team is registered.
COACH'S CLEARANCE GUIDE
The items listed are what each coach must provide or do for registration.
PA CRIMINAL CHECK (SP4-164)
Each person must have a valid PA Child Abuse Form, SP4-164. Although not done annually, it cannot be more than 5 years old. He should keep the original and give a copy to the head coach.
PA CHILD ABUSE FORM - CY113
Each coach must have a valid PA Child Abuse Form, CY113. Although not done annually, it cannot be more than 5 years old. He should keep the original and give a copy to the head coach.
AFFIRMATION OF VOLUNTEER STATUS
To be signed by all coaches and presented to the team's administrator.
ABUSE TRAINING (SENATE BILL 534)
To be taken by all coaches. He should keep the original and give a copy to the head coach.
COACHES ROSTER
Lists all personnel directly associated with the team's players.
