SPORTSPLEX – PROOF OF VACCINATION POLICY
Beginning Wednesday, September 22, 2021, all patrons entering the Sportsplex facility are required to provide proof of identification and proof of being fully vaccinated against COVID 19. This is as per the provincial restrictions outlined in section 2.1 of Schedule 1 of O.Reg.364/20: Rules for Areas at Step 3 and at the Roadmap Exit Step. Should an individual choose not to share this information, then they will not be permitted to enter. It will be the responsibility of the person renting the ice to ensure that all patrons entering the Sportsplex are fully vaccinated. The Municipality of Powassan will be performing random checks of all users to make sure that the policies are being followed.
Please review the following information carefully:
Proof of Vaccination must be provided by:
All patrons/players/walkers 18 years of age or older
All spectators coming to view a game/practice/event who are 12 years of age or older
The following exemptions apply:
Workers, contractors, repair workers, delivery workers, inspectors or others who are entering the arena for work purposes and not as patrons
A patron who is entering to make a retail purchase or purchase admission
Children under 12 years of age
Children aged 12-17 who are entering the arena to participate in a scheduled game/practice, not as a spectator of said game/practice
Patrons who provide identification and a written document completed and supplied by a physician (MD) or a registered nurse in the extended class (RNEC, NP) stating that the individual is exempt for a medical reason from being fully vaccinated against COVID 19
Vaccine Certification Process
At the time of vaccination, you would have received a receipt on site or via email. This receipt includes your name, date of birth, the date of the vaccination and the product name of the vaccine received. Copies of these receipts can be downloaded or printed through the provincial site by logging on to https://covid-19.ontario.ca/ and clicking on “Get Proof of Your Vaccination”. The receipt provided must show that the patron is fully vaccinated and that their final dose was administered at least 14 days prior to the current date.
Appropriate Forms of Identification:
The identification provided must include the patron’s name and date of birth. Appropriate forms of identification include:
Birth Certificate Citizenship Card
Driver’s License Health Card
Indian Status Card/Indigenous Membership Card Passport
Permanent Resident Card
**The name and date of birth on the receipt must match the name and date of birth on the piece of identification provided, or the patron cannot enter.
Beginning October 22, 2021, individuals will be able to return to the provincial site and download a QR Code which will contain their vaccination information, rather than supplying paper copies of the vaccination receipts; however both will continue to be accepted.
Please note: Current capacity limits, patron screening, masking, physical distancing and collection of patron contact information will continue to be required as previously indicated.
