2019 Lower Nazareth Baseball & Softball Clearance Process

If you are interested in coaching or assisting on the field during games or practices, you must complete the 2019 clearance forms listed below. 

Please note that you WILL NOT be allowed on the fields with the kids without the necessary clearances!

The following 3 items need to be completed every year:

1) Lower Nazareth Township Certified Sports Volunteer Application

2) Little League Volunteer Application Form

3) Copy of your state driver's license or government issued ID (with current address)

The following needs to be completed once every five years:

1) PA State Police Background Check (online)

• NOTE:  Please print out several copies of this clearance form.  You will need to submit it to the township with each sport for the next 5 years, or to any other volunteer organization you will encounter.  I believe once the form closes, you will no longer have access to print it.  The cost is FREE, if and only if you click the Highlighted box in the middle of the page that states "VOLUNTEER." This is only free once every 57 months, so do not lose your copies.

• NOTE:  You will need to create an account prior to doing the check. This check is FREE once every 57 months.   Once your application is processed, you ideally should be able to print this out immediately, or within a few days.  Again, please print several copies of this clearance form, as you will need to submit a copy for each organization you plan on volunteering for.  I believe you may be able to print out additional copies later by signing into your account, but I am not positive, so best to be safe and print several copies.

• That process takes several weeks so I would begin this process ASAP
