Waiver Form

Instructions: Players are required to read the ‘Waiver, Release and Indemnity’ form as well as the KPSHL Constitution and then the players are to sign the roster form provided indicating that they have read and will abide by the rules contained in said documents. Failure to comply may result in penalties as indicated in the rules section.

Waiver, Release and Indemnity

  In consideration of the acceptance of my application herein and the permission to participate as an entrant or competitor I the Kingston Public Service Hockey League in Ontario under the rules of the Canadian Hockey Association.

I, for myself, my heirs, executors, administrators, successors and assigns HEREBY RELESE, WAIVE AND FOREVER DISCHARGE any and all claims for damages I may have against the Kingston Public Service Hockey League and all other associations, contractors, representatives, volunteers, sponsors, successors and assigns and against the City of Greater Kingston and the province of Ontario and their sponsors, successors and assigns for any and all injuries, loses and damages to my person or property that I may suffer as a result of my participation herein, whether prior to, during or subsequent to the event herein, and not withstanding that the same may have been contributed to or occasioned by negligence of any of the aforesaid persons or entities.

 I acknowledge that participating in this event carries with it potential hazards and that I am physically fit and have ability to participate in this event.

 I further hereby undertake to hold and save harmless and agree to identify all of the aforesaid persons and the entities from and against any and all liability incurred by any or all of them arising as a result of or in any way connected with my participation in the league.

 By submitting this entry, I acknowledge having read, understood and agree to the above waiver, release and indemnity.
