2024 TRYOUT Registration for the Iowa Great Lakes American Legion Baseball Team

We appreciate your patience as you review the below two waivers. We utilize many facilities, fields, and gyms and all have their own rules and requirements. These waivers must be completed before attending the player's first tryout. Thank you.

Waiver:

In consideration of the permission granted to me/my child/ward to participate in the year 2024 Total Baseball Development LLC (TBD) activities schedule, I individually or as a parent/guardian hereby release, covenant not to sue and forever discharge, Robert Gerald Protexter; Total Baseball Development LLC; 90TEN Athletics in Pocahontas, Iowa & Owner/Operator Cody Bunda; American State Bank Sports Complex in Sioux Center, Iowa; the TBD Fall League; The TBD Futures League; Milford American Legion Post #384 (Iowa); Iowa Great Lakes 'Okobojis' American Legion Baseball Team; The City of Sioux City, Iowa; The Sioux City Community School District; The Sioux City Parks and Recreation Department; The Sioux City Explorers Baseball Club; Lewis & Clark Park; Little League Baseball; Morningside Little League Inc.; The Strikers Complex; Sioux City East High School; Sioux City North High School; Sioux City West High School; Sergeant Bluff-Luton Community School District; Hinton Community School District, Briar Cliff University, Morningside College, The City of Lake Park, Iowa, The Harris-Lake Park Community School District, The City of Spirit Lake, Iowa, The Spirit Lake Community School District; The City of Milford, Iowa; the Okoboji Community School District; Vosburg Field in Granville, Iowa; Kenny Burn Field in Cherokee, Iowa; Wildcat Stadium and the City of Arnolds Park, Iowa; and the Graettinger-Terril-Ruthven-Ayshire School Districts; and any other Facility, City, Township, School District, individual high school, high school booster clubs, independent contractors, and their Associations, Organizations, and related parties; and any officers, employees, or agents of any of the aforementioned groups, associations, or organizations from any and all claims and liabilities with regard to my or my child's/ward's participation in the 2024 Total Baseball Development LLC (TBD) activities schedule.

I (individually or as a parent/guardian) further state and certify that I am able to participate in the described activities of the camp and league and activities. I/my child/ward further agree that should I/my child/ward become injured as a result of participation that I/my child/ward do release and hold harmless, Robert Gerald Protexter; Total Baseball Development LLC; 90TEN Athletics in Pocahontas, Iowa & Owner/Operator Cody Bunda; American State Bank Sports Complex in Sioux Center, Iowa; the TBD Fall League; The TBD Futures League; Milford American Legion Post #384 (Iowa); Iowa Great Lakes 'Okobojis' American Legion Baseball Team; The City of Sioux City, Iowa; The Sioux City Community School District; The Sioux City Parks and Recreation Department; The Sioux City Explorers Baseball Club; Lewis & Clark Park; Little League Baseball; Morningside Little League Inc.; The Strikers Complex; Sioux City East High School; Sioux City North High School; Sioux City West High School; Sergeant Bluff-Luton Community School District; Hinton Community School District, Briar Cliff University, Morningside College, The City of Lake Park, Iowa, The Harris-Lake Park Community School District, The City of Spirit Lake, Iowa, The Spirit Lake Community School District; The City of Milford, Iowa; the Okoboji Community School District; Vosburg Field in Granville, Iowa; Kenny Burn Field in Cherokee, Iowa; Wildcat Stadium and the City of Arnolds Park, Iowa; and the Graettinger-Terril-Ruthven-Ayshire School Districts; any other Facility, City, Township, School District, individual high school, high school booster clubs, independent contractors, and their Associations, Organizations, and related parties; and any officers, employees, or agents of any of the aforementioned groups, associations, or organizations from any and all liability for illness or injury and any consequences thereto and there from. This release shall insure to the benefit of Robert Gerald Protexter; Total Baseball Development LLC; 90TEN Athletics in Pocahontas, Iowa & Owner/Operator Cody Bunda; American State Bank Sports Complex in Sioux Center, Iowa; the TBD Fall League; The TBD Futures League; Milford American Legion Post #384 (Iowa); Iowa Great Lakes 'Okobojis' American Legion Baseball Team; The City of Sioux City, Iowa; The Sioux City Community School District; The Sioux City Parks and Recreation Department; The Sioux City Explorers Baseball Club; Lewis & Clark Park; Little League Baseball; Morningside Little League Inc.; The Strikers Complex; Sioux City East High School; Sioux City North High School; Sioux City West High School; Sergeant Bluff-Luton Community School District; Hinton Community School District, Briar Cliff University, Morningside College, The City of Lake Park, Iowa, The Harris-Lake Park Community School District, The City of Spirit Lake, Iowa, The Spirit Lake Community School District; The City of Milford, Iowa; the Okoboji Community School District; Vosburg Field in Granville, Iowa; Kenny Burn Field in Cherokee, Iowa; Wildcat Stadium and the City of Arnolds Park, Iowa; and the Graettinger-Terril-Ruthven-Ayshire School Districts; any other Facility, City, Township, School District, individual high school, high school booster clubs, independent contractors, and their Associations, Organizations, and related parties; and any officers, employees, or agents of any of the aforementioned groups, associations, or organizations and shall be binding on my heirs, successors and executors. I/my child/ward further state that Total Baseball Development LLC is authorized to use my/my child/ward's name and any photographs or videotape of me/my child/ward at the camp and league and activities for its promotional purposes without the need to compensate me/my child/ward or Total Baseball Development LLC for such use.

I have read this liability form and have been given sufficient time to review it and ask whatever questions I have relating to it. I fully understand the risks involved and that it is possible to sustain serious injury or death during the course of participating in the above-described activities. I acknowledge that my execution hereof is material to acceptance of my participation in the 2024 Total Baseball Development LLC (TBD) activities schedule.
