HOOPS BY THE SEA PLAYER REGISTRATION FORM 2026 as of March 1st

This form will began the registration process for your player for our 2026 summer basketball league "Hoops by the Sea"
However, completing this form doesn't guarantee your child a spot in the league. Due to limited spots we fill up the age groups in the order we receive the registration form online. This form is for players currently in grades 2nd to 7th who will be entering grades 3rd to 8th in the fall of 2026. We will send an invoice for $145 within 7 days of completing this form if we have room in your child's age division. If not you will receive an email that your child has the option to be placed on our waitlist. We look forward to summer sun and outdoor basketball!!!

WAIVER INFORMATION
I hereby agree to permit my child to participate in Hoops by the Sea.
I acknowledge that I have been advised of medical risks that may result in such
participation and I represent that I have consulted my child's physician
and my child is physically capable of such participation without injury.
I hereby waive and release Narragansett Youth Sports and
the Town of Narragansett from any claims, liabilities, and expenses
arising from my participation in the above the program.

By signing this document, I also agree that if I am exposed or infected by COVID-19 during my participation in this activity, then I may be found by a court of law to have waived my right to maintain a lawsuit against the parties being released on the basis of any claim for negligence.
