Over 35 Application for draft

This form is to be filled out by any player that would like to play in the over 35 league that played in the SMISL under 35 league in 2016, is under the age of 40, and that turns 35 before May 25th, 2017

You must submit this form to the league by May 15th to be entered in the draft.

You must register with the Over 35 league first in order to be eligible to be drafted.
