Aerial Fusion Baton Registration Form




I acknowledge that certain risks of injury are inherent to participation in baton. These types of injuries may be minor or serious and may result from one’s actions, or the actions or inactions of others or a combination of both. I understand that certain activities require a minimum level of fitness and health (physical, mental and emotional and that each person has a different capacity for participating in these activities. Some activities involve an intense level of activity. I agree that Aerial Fusion Baton and staff shall not be liable for any injury, loss or damage to person or property, incurred during this program, including deterioration of health or illness or aggravation of any pre-existing condition resulting from participation in these activities, property damage or lost property.
Expectations: Proper attire is necessary to ensure the athlete gets the most out of class and to prevent injury. Athletes are required to come to class in appropriate footwear, tights, body suit and hair up and secured. Failure to come to class appropriately dressed may result in the athlete being removed from class.
Behaviour: In order for each athlete to reach their full potential, respect is essential, and we need both athlete and parents’ cooperation and good behavior. Any parent or student acting in a disrespectful manner, or creating conflicts or making derogatory comments towards other parents, students, teachers, or other clubs and athletes, will be strictly dealt with, and may result in the student being removed from class permanently.
Photography/Video/ Website/ Publicity:
I agree that my child’s picture may be used for promotional materials, including printed and website material, and that my
child may video-taped at the Christmas performances, competitions and year-end recitals.
CLASSES (Check all that apply)
Recreational Program (45 minutes)
Pre-Competitive Program includes weekly training in preparation for MBTSA competitions. May include some additional lessons throughout the year. Participation in the Pre-Event Challenge is expected as well.
Competitive Program is for athletes in at least 2 competitive events. Participation in all MBTSA competitions and at Canadian Championships is mandatory.
Group Events - Participation in at least 1 group event is expected

Payment information:
1) Please make cheques payable to Aerial Fusion Baton.
2) Pre-Comp: Please pay in 7 post-dated cheques dated the 1st of every month (Oct to April)
3) Competitive: Please pay in 8 post-dated cheques dates the 1st of every month (Oct to May)
4) Missed lessons without prior notice may be assessed a credit
5) Any missed private lessons will be reconciled in May
6) June fees, for athletes attending Canadians, will be assessed separately
7) Pre-Comp and Competitive - there is an additional fee for membership with the Canadian Baton Twirling Federation. Please see coach for details if you have not received your forms.
8) Registration and Gym Fee is due upon registration
