2023 Referee Evaluation Form

Referee Scorecards must be submitted by Monday following the evaluated game.
All scorecards should be completed by the Commissioner, Head or Assistant Head coach.







If you need to submit a video please send via email directly to Rick Basurto at rick.rquad@gmail.com after completing this form.
All submissions are reviewed by the Conference Commissioner during weekly calls with the Referee Coordinator.

Make sure you enter the Submit button at the end of the survey. This will take you to the confirmation page.
