I hereby release Erik Lovdahl, Lovdahl Pitching Academy, and LPA Baseball, and all employees from all claims on account of any injuries which may be sustained by my child while attending the camp/clinic and any claims hereafter which may be presented by our child as a result of any such injuries. I also certify that my child is medically fit to participate in this camp/clinic. I hereby authorize the directors of Lovdahl Pitching Academy to act for me according to their best judgement in any emergency requiring medical attention.
