This form is for any and all sponsorships and/or donations made to the SkyHawks Baseball Club for the 2019 baseball season. Please print two forms for every one sponsorship and/or donation.
All forms must be filled out in duplicate.
One copy to sponsor/Donor and one to the SkyHawks.

The SkyHawks Baseball Club, a member of the Roy Hobbs Adult Amateur Baseball League and the Roy Hobbs Northwest Ohio Baseball League, is solely dependent on sponsorships and donations for our funding. All funds are dispersed for league fees, umpire fees and equipment. The total cost for the 2019 RHNWOBL season is $2,450.00. Your help with all or any part of this cost through sponsorship or donation, is greatly appreciated.
