2025 Lisle Baseball / Softball Fall Ball Registration Please read the following waiver, the form will ask for you to check that you have read and agree: This is to certify that as parent or guardian of the player I am registering with Lisle Baseball-Softball League, Inc. I do hereby grant permission to an adult manager or coach of the team to obtain medical care from any licensed physician, hospital or medical clinic for the player named in registration form at such time as either the parent or legal guardian cannot be contacted in person or by telephone. This authorization shall include all league activities, including the period required to travel to and from said activities. I also hereby further waive, release, absolve, indemnify and agree to hold harmless Lisle Baseball-Softball League, Inc., the organizers, supervisors, participants, managers, coaches, league directors and officers, and persons transporting players to and from league activities for any claim arising out of an injury to the player. |
