The Penncrest High School Daily Health Assessment Form (link below) must be completed by a parent/guardian prior to EVERY practice/competition your athlete attends. Please do not submit this form any earlier than ONE HOUR prior to your child's attendance at each event. Your athlete may NOT participate in the practice/competition unless this form is submitted each time she is present. (Please click on link)
