• Registration Form / Photo Release Form
• Consent, Acknowledgement of Risk, Waiver of Liability and Indemnification Agreement
  
• Authorization for Emergency Treatment

Registration Fee: 

• $10.00 per session paid at the door.
• One Payment - $60.00 for all 7 sessions. (Get 1 Sessions FREE!)
• One Payment - Get another girl from your school and register for all 7 sessions, each girl pays $50.00. (Get 2 Sessions FREE!)

Note: Fees paid by check should be made out to Backyard Training Academy.

We suggest that you bring your own Water Bottle (for water breaks).

And of course, don't forget...your Sneakers. 😊
